However, the facility was unable to provide a 5-day report or an investigation for an incident involving a resident who was admitted with heart disease, atrial fibrillation, bipolar disorder, anxiety, and dementia and had a BIMS score of 3 indicating severe cognitive impairment. A progress note documented that the resident was hit by another resident on the right side of the neck and had a red area to that area, and the Intake Status Report stated the resident was allegedly hit 3 or 4 times in the head, neck, and back area. The facility was also unable to provide a 5-day report or an investigation for another resident who was admitted with diabetes, dementia, heart failure, anxiety, and chronic kidney disease and had a BIMS score of 5 indicating severe cognitive impairment. The Intake Status Report documented that this resident was allegedly hit by another resident and then hit back, and the care plan later noted the resident had the potential to demonstrate physical behaviors and referenced the resident striking another resident. During interview, the Administrator, who was also the Abuse Coordinator, confirmed she could not provide documentation of a 5-day follow-up or evidence that a thorough investigation had been completed for either incident.
