The facility failed to properly store medications and medical supplies in 1 of 2 medication carts observed for medication storage. Review of the facility policy titled, Medication Storage in the Facility, showed that medications and biologicals are to be stored safely, securely, and properly, that drugs in the manufacturer's original container expire on the manufacturer's expiration date, and that expired medications are to be removed from the active supply. Resident #49 was admitted with diagnoses including Essential Hypertension, Gastro-Esophageal Reflux Disease, and Low Back Pain, and had an order for ondansetron 4 mg every 6 hours as needed for nausea/vomiting. Review of the MAR from 3/1/2026 through 6/9/2026 showed the resident had not received ondansetron 4 mg. During observation of the 200 long hall medication cart with an LPN, surveyors found an ondansetron 4 mg tablet for Resident #49 with an expiration date of 12/3/2025, an unopened package of 6 breathable adhesive skin closures, an opened package with 3 breathable adhesive skin closures with an expiration date of 4/16/2026, (3) 20 gauge by 1-inch IV needles with an expiration date of 3/2/2025, and (1) 20 gauge by 1-inch IV needle with an expiration date of 12/4/2025. The LPN confirmed the expired ondansetron, skin closures, and IV needles were available for resident use.
