The facility failed to provide a completed Bed-hold document for Resident 2 and/or the resident’s responsible party within a practicable amount of time. The facility policy titled, Bed Hold Prior to Transfer, required written information about bed-hold policies to be given to the resident and/or representative prior to and upon transfer to the hospital, including the bed-hold amount. Resident 2 was admitted to the facility with diagnoses including dementia, chronic kidney disease with heart failure, and sepsis with severe septic shock, and later had a hospital stay beginning on 10/14/2025. Review of the transfer and bed-hold documents showed that a bed-hold form was mailed to the responsible party on 10/20/2025, six days after the resident was sent to the hospital, but the form was blank and did not document the bed-hold amount. No documentation was found to show that the corrected bed-hold information was mailed with the amount included before the resident returned to the facility. During interview, the Admissions Coordinator stated that she realized after mailing the form that she had not filled out the bed-hold amount and that this was an oversight on her part. The DON stated she would expect the bed-hold information to be mailed out timely to the resident representative.

The resident was admitted with diagnoses including dementia, congestive heart failure, and sepsis, and the record showed an admission for hospice care and services. Review of the facility hospice policy and the Nursing Facility Hospice Services Agreement showed that hospice and facility staff were expected to communicate, coordinate care, and document communications in the resident’s clinical record. The medical record for the resident was missing routine nursing assessments and supervisory visits from hospice registered nurses on multiple dates in December 2025. There was no documentation in the medical record showing coordination of care from hospice nurses to facility nurses and staff, and hospice nurses did not convey any changes or findings from the visits. During interviews, the Clinical Coordinator provided faxed hospice assessments for the missing nurse assessments, while the Unit Manager stated hospice staff usually speaks to the nurse before leaving but could not provide documentation of who was spoken to or what information was communicated. The DON stated she would expect documentation in the medical record concerning coordination of care by the hospice nurse and the facility nurse caring for the resident.
