Medication was not securely stored for one resident when a dose of omeprazole was left in an unmarked medication cup on top of an unattended medication cart in the hallway. The resident had been admitted with a history of GERD without esophagitis, and the active order was for omeprazole delayed release capsule 20 mg once daily at 6:00 AM. During observation, RN1 was away from the cart assisting another resident, and the medication remained on the cart until the nurse returned several minutes later and placed the unmarked cup back into the cart drawer. The nurse stated the medication was omeprazole for the resident and explained that she had left it on the cart because she was busy assisting a resident with a call light. RN1 stated she could not throw out the medication because she would need the next dose and the packets were replaced weekly, and she planned to give it later that morning. During later observation, RN1 removed the previously opened medication from the unmarked cup and administered it to the resident. The UM, IP, and DON stated that opened medication should not be returned to the cart and should be disposed of, and the IP stated staff received yearly skills training regarding medication administration and storage.

RN1 entered the room wearing only a surgical mask and gloves, and stated she was only required to wear a mask and gloves. CNA2 also entered the room wearing only a mask and gloves and stated the same. The Infection Preventionist stated the resident had droplet precautions and staff were expected to enter the room wearing PPE, including a gown, gloves, mask, and face shield or goggles, and noted goggles were not available at the resident's door even though they were available in central supply. The DON stated he expected staff to wear the appropriate PPE.
