Review of the September 2025 physician orders and MAR showed Bupropion 100 mg was ordered three times daily at 6:00 AM, 2:00 PM, and 10:00 PM, and Gabapentin 100 mg was ordered three times daily at 6:00 AM, 2:00 PM, and 10:00 PM. The LPN stated she gave both medications around 8:30 AM and could not verify the electronic record times. The DON later provided the MAR and physician orders and stated both medications should not have been given during that medication pass because they had already been administered at 6:00 AM by the night shift nurse.
