The worker stated soiled linen should have been placed in plastic bags before leaving resident rooms and that it is often thrown into bins without bagging.

Improper medication storage was identified when surveyors observed loose pills in 3 of 3 medication carts. The facility policy titled Medication Access and Storage, E Kit Access, revised 07/2022, stated that medications are to be kept and stored in their containers and that compromised or deteriorated medications or med packs are to be discarded upon discovery. During observation of the North 1 medication cart, six loose pills not packed for resident use were found. An LPN confirmed the medications and removed them from the cart. During observation of the East 1 medication cart, one loose pill not packed for resident use was found along with a Ziploc bag containing nine 1 oz packets of Pro-stat concentrated liquid protein 15 g protein in 1 fl oz, with liquid in the bottom of the bag. During observation of the South 2 medication cart, five loose pills not packed for resident use were found. In each instance, the medications were confirmed by nursing staff and removed from the cart. Interviews with LPNs and the DON showed differing descriptions of cart-cleaning practices, including daily, nightly, weekly, and end-of-shift cleaning, and all stated that loose pills should be discarded when discovered.

Provide and implement an infection prevention and control program was deficient because the facility did not follow its process for handling and transporting soiled linen to reduce the possible spread of infection. Review of the facility policy titled, Departmental (Environmental Services) - Laundry and Linen, stated that all soiled linen must be placed directly into a covered laundry hamper and transported in a bag to the soiled utility room. During observation on 03/18/2026 at 8:20 AM, Laundry Worker 1 was seen pushing a large covered bin to pick up soiled linen from Hall 2 and then entering the soiled utility room on Hall 200, where the soiled linen carts were overflowing. Very few bags of soiled linen were in the bin, and most of the soiled linen was unbagged and thrown into the bin, including linens soiled with feces and urine. During interview at approximately 8:25 AM, Laundry Worker 1 stated the soiled linen should have been placed in plastic bags before leaving residents' rooms and said that most of the time the soiled linen is just thrown into the bins and is hardly ever bagged before being taken to the soiled utility rooms.
