The resident's care plan indicated the need for hand splints to be worn when up in the wheelchair, but staff members, including a PT, OT, CNA, and RN, were either unaware of this requirement or had never seen the resident wearing the splints. This oversight was confirmed by the Director of Nursing Services and a Nurse Manager, who acknowledged that the resident's care plan should have been followed to prevent contracture progression. Observations and interviews revealed that the resident's fingers were curled into the palms, indicating a worsening of the contractures. Despite the care plan and therapy notes specifying the use of hand splints, direct care staff did not implement this aspect of the resident's care. The failure to adhere to the care plan and ensure the resident wore the hand splints as required placed the resident at risk for worsening contractures, as confirmed by multiple staff members and the resident's care documentation.
