The DON reported that some residents were dependent on staff for eating, and the facility policy stated residents should not be stood over while being assisted with meals.
Meals Served Cold: The facility failed to ensure meals from the kitchen were served at an appetizing temperature for 54 residents. A resident with intact cognition and independence with eating reported food was often served cold, including all meals over one weekend, and soup had to be sent back three times to get hot enough. Multiple residents in a council meeting agreed the food was cold that weekend and sometimes cold during the week, and the DON and dietary manager were aware of the complaints.
Infection control was not followed during meal assistance for three dependent residents. A CNA was observed blowing on a resident’s spoon, touching their face and hair, handling food with bare hands, and moving between residents without hand hygiene while assisting with feeding. The DON stated the residents were dependent on staff for eating, and the administrator identified the observed practices as infection control concerns.
