The facility failed to ensure that meals served from the kitchen were at an appetizing temperature for all residents receiving meals, with the DON reporting that 54 residents received meals from the kitchen. Resident #64 had intact cognition with a BIMs score of 15 and was independent with eating. Resident council minutes from July 2025 documented complaints about cold food. The resident reported that food was served cold often, and stated that over the weekend of 08/16/25 and 08/17/25 all meals were cold and the soup had to be sent back three times to be hot enough. The resident ate meals in the dining room. During a resident council meeting, multiple residents agreed that food served that weekend was cold and was sometimes cold during the week, and they stated there were too many residents in the dining room for staff to reheat all meals at the same time. The dietary manager reported being aware of complaints about cold food on weekends and stated that three new kitchen staff had been hired for weekends.
