Stored bread and other opened food items were found without proper dating during the initial kitchen observation. The dietary manager observed multiple half-full, undated items, including 2 bags of hamburger buns, 2 bags of hot dog buns, 1/2 bag of chips, 1 and 1/2 loaves of white bread, and 1/2 bag of English muffins. The dietary manager stated the policy and procedure was to label food when opened or when it comes in, and acknowledged that none of the identified bread was dated. Both the large and small ice machines were observed with debris and signs of inadequate cleaning. The large ice machine had brown/black debris on a white plastic piece inside the machine, and there was no signage showing when it was last cleaned; the dietary manager stated there was black material and that it was dirty, and said they were not aware of who was responsible for cleaning it. The small ice machine had floating gray debris and a dead moth in the water at the bottom of the drip tray, and there was no cleaning log near the machine. During meal service, cook #1 plated food while wearing the same gloves, then handled an open bag of tortillas, touched ladles and plates, and continued using the same gloves. The dietary manager stated the cook should have used tongs to pull the tortillas and acknowledged not noticing that gloves were not changed before the cook touched the ladles again.

The facility failed to ensure infection prevention and control practices were followed for a nebulizer mouthpiece used by a resident with COPD. A nebulizer machine with a mouthpiece was observed on top of the resident’s drawer, with the mouthpiece set on the drawer. The resident had a physician’s order for ipratropium/albuterol inhalation solution four times daily for COPD, and the resident stated they used the nebulizer daily. The facility’s policy stated staff involved in administration must follow infection control procedures and adhere to the treatment protocol to prevent cross-contamination and ensure efficacy. During interview, a CMA stated the mouthpiece should be stored in a bag or resident drawer when not in use, while the DON stated the mouthpiece was to be set on the nebulizer machine. The facility also failed to store clean linens appropriately in the laundry room. Two grey barrels lined with plastic bags and lids were observed next to a clean clothing rack, and clothing items on the rack were touching the floor. A short clothing rack in the clean storage area also had clothes touching the floor, and five bags were sitting on the floor in the clean storage area. The laundry supervisor identified the clothing on the racks as clean, stated the barrels were used for collecting dirty linens and were not 100% clean, and stated they were not stored in a manner that prevented cross contamination. The supervisor also stated the clothes on the short rack were not stored appropriately and that the bags on the floor were not stored appropriately.
