Infection control practices were not maintained for a resident on contact isolation for C. diff. A COTA was observed leaving the resident's room and used hand sanitizer instead of washing with soap and water, then touched another resident's door handle while carrying linens. The DON confirmed soap-and-water handwashing was required for C. diff, and the facility policy and CDC guidance both addressed hand hygiene after PPE removal and before leaving the room.
A facility failed to ensure a deceased resident’s personal funds were conveyed for final accounting within 30 days of death. The resident had chronic pulmonary disease, DM, and kidney disease, and account records showed a balance of about $300 that was later written to the Attorney General office to close the account. The BOM verified the delay.
Failure to Address PTSD in Care Plan: A resident with PTSD, Parkinson's disease, Alzheimer's disease, HTN, and HF had PTSD identified on the MDS, but the comprehensive care plan did not address it. Staff reported flashbacks, tearfulness about the war, depression, and anxiety, and a psych eval added PTSD as a diagnosis. Interviews confirmed there was no PTSD assessment available and the resident's PTSD was not included in the care plan despite the facility's trauma-informed care policy.

Penalty
Summary
Provide and implement an infection prevention and control program was not maintained for a resident in contact isolation for C. diff. Resident #86 was admitted with diagnoses including traumatic subdural hemorrhage, pneumonia, and dementia, and was placed on contact isolation on 06/16/26 for C. diff. The COTA then touched another resident's door handle while carrying a bag of linens and stated she did not know she had to wash her hands instead of using hand sanitizer. On 06/25/26 at 1:50 P.M., the DON stated hands must be washed with soap and water for C. diff because hand sanitizer did not work. The facility policy stated hand hygiene must be performed after PPE removal and before leaving the room, and CDC guidance stated washing hands with soap and water was the best way to prevent spread of C. diff.
Failure to Timely Convey Resident Funds After Death
Penalty
Summary
The facility failed to ensure resident funds were conveyed for final accounting within 30 days of death for Resident #89. Review of the medical record showed the resident was admitted with chronic pulmonary disease, diabetes mellitus, and kidney disease, and later expired at the facility. Review of the resident’s personal funds account statements showed a balance of $300.06 on one date and $300.17 on a later date. A check dated after the resident’s death showed $300.17 was written to the Attorney General office to close the account, and the Business Office Manager verified the lack of timely conveyance of the resident’s funds.
Failure to Address PTSD in Care Plan
Penalty
Summary
The facility failed to ensure a care plan was initiated for a resident with post-traumatic stress disorder (PTSD). Resident #8 was admitted with diagnoses including PTSD, Parkinson's disease, Alzheimer's disease, hypertension, and heart failure. The resident's MDS assessment identified PTSD as a diagnosis and noted cognitive impairment, but the comprehensive care plan initiated on 07/19/22 did not address PTSD. A psychiatric evaluation dated 12/18/25 documented that staff reported the resident was having flashbacks and had been more tearful about the war, with increased symptoms of depression and anxiety; PTSD was added as a diagnosis. During interviews, MDS nurses stated PTSD would be addressed in the admission assessment, while the MSW stated the resident initially denied PTSD on admission and later discussed war-related concerns, which led to a psychiatric referral. The MSW verified there was no PTSD assessment available under the Assessment tab and confirmed the resident's PTSD was not addressed in the care plan. The facility policy on Trauma Informed Care stated residents with signs or symptoms of trauma were to be closely monitored and have trauma-informed care plans produced, documented, and implemented.
