Failure to Maintain Safe, Clean Environment and Functional Ventilation
E
F0584 F584: Honor the resident's right to a safe, clean, comfortable and homelike environment, including but not limited to receiving treatment and supports for daily living safely.
Short Summary

Surveyors found that the facility did not maintain a safe, clean, and homelike environment in multiple rooms and bathrooms. Boxes of medical and cleaning supplies, including disposable nebulizers, Micro-Kill One wipes, drain sponges, bed pans, and cleansing wipes, were stored outside on a pallet and covered with snow. In several resident areas, a baseboard radiator cover was loose with exposed fins, a shared bathroom faucet was heavily corroded and flaking, and a four-receptacle electrical outlet had a cracked cover exposing the interior of the electrical box. Additional findings included heavily bent and damaged baseboard heater covers, a bathroom door that was difficult to open due to catching on the flooring, nonfunctional exhaust ventilation in several shared bathrooms, and a strong urine and feces odor with two full garbage bins in one shared bathroom.

Failure to Maintain Sanitary and Homelike Environment; Inadequate Bathing Facilities
F
F0584 F584: Honor the resident's right to a safe, clean, comfortable and homelike environment, including but not limited to receiving treatment and supports for daily living safely.
Short Summary

Surveyors found that the facility did not provide adequate housekeeping and maintenance, resulting in persistent odors, dirty floors, chipped paint, and unsafe conditions such as a protruding nail and nonfunctional soap dispensers. Equipment like apex machines and the dumbwaiter were observed to be dirty and improperly maintained. The only shower room was out of service for an extended period due to subfloor damage, with residents receiving bed baths and a portable shower sink as alternatives. Staff confirmed inconsistent cleaning practices and acknowledged the environment was not homelike.

Failure to Maintain Resident Dignity and Provide Timely Incontinence Care
E
F0550 F550: Honor the resident's right to a dignified existence, self-determination, communication, and to exercise his or her rights.
Short Summary

A resident with cognitive and physical impairments was left in the dining room for hours with a strong odor of bowel incontinence, without timely incontinence care, and was later found with a saturated brief and reddened skin. Multiple residents were observed wearing hospital gowns in public areas instead of personal clothing, and staff were observed speaking to a resident in a loud, demeaning manner when the resident requested to return to their room.

Failure to Implement Infection Control Practices and Enhanced Barrier Precautions
E

Short Summary

Surveyors found that staff failed to follow infection control protocols, including proper hand hygiene after incontinence care and the use of Enhanced Barrier Precautions (EBP) for residents with wounds. In two cases, staff did not remove gloves or wash hands after providing care and did not use required PPE such as gowns during high-contact activities, leading to cross-contamination and non-compliance with facility policy.
