Summary

Surveyors identified that the facility failed to maintain a safe, clean, comfortable, and homelike environment in multiple resident rooms and a ventilation system. Observations over several days showed approximately eight boxes of Medline supplies, including disposable nebulizers, Micro-Kill One wipes, drain sponges, bed pans, and dry cleansing wipes, stored outside on a wooden pallet and covered with snow near the loading dock. In one resident room, a baseboard radiator cover had come loose, exposing about a one-foot section of radiator fins. In another shared bathroom serving two resident rooms, the sink faucet was heavily corroded and flaking. In a different room, the cover plate for a four-receptacle electrical outlet below the center television was cracked and damaged, exposing the interior of the lower electrical box. Another room had baseboard heater covers that were heavily bent, chipped, and damaged, and the bathroom door in that room was very difficult to open because it caught on the flooring. Additional observations showed that exhaust ventilation was not functioning in several shared bathrooms, including those for rooms #11 and #13, #14 and #16, and #21 and #23, with no air being drawn through the ceiling exhaust grates. In the shared bathroom for rooms #11 and #13, there was a significant foul odor of urine and feces, and two full garbage bins were present. During interviews, the Director of Environmental Services indicated the outdoor supplies had likely been delivered several days earlier, and the Director of Maintenance reported that a vendor was onsite repairing the rooftop exhaust unit because it was not receiving enough power to run properly. These conditions were cited as failures to provide necessary housekeeping and maintenance services under 10 NYCRR 415.29 and related subsections.
