Resident #3, with severe cognitive impairment and dependence on staff for all ADLs, had missing records for bed mobility, turn and position, and eating. Interviews with facility staff, including CNAs and nursing management, confirmed that CNAs were responsible for documenting ADL care by the end of each shift, yet the documentation was incomplete, violating the facility's policy on charting and documentation.
