Review of the Water Management Plan dated 1/1/26 showed the facility relied on CDC guideline materials and included monthly visual inspections of sinks, showers, holding tanks, and mixing valves, monthly inspection and maintenance of thermal mixing valves, monthly flushing of low-use water systems, and annual testing by an outside contractor. However, the plan did not contain a risk assessment, and the control measures were generic rather than tied to specific locations or interventions when controls were not met. The plan also did not include control measures for empty resident rooms. During interview, the Maintenance Director confirmed that the plan did not specify control measures or specific interventions when measures were not met and stated there was no documentation of interventions for out-of-range control measures. The Maintenance Director was also unable to produce evidence of annual water testing for 2025 and was unaware whether it had been completed. The Social Worker reported that the facility had 2 empty resident rooms that had been vacant for greater than 7 days, and the Director of Environmental Services stated that housekeeping handled terminal cleaning when a room became empty but did not perform other activities such as flushing faucets.
