Review of the facility policy titled, Controlled Substance Administration & Accountability, showed controlled substances are to be stored in a separate compartment of the medication cart or refrigerator lock box, and two licensed nurses are to account for all controlled substances and access keys at the end of each shift. During observation of the medication storage area, a locked box in the medication refrigerator contained three syringes of Ativan, one box of oral Ativan with a resident label, and one box of oral Ativan as part of the emergency medication kit. The outside of the emergency medication kit listed medications and expiration dates, including a Lupin inhaler with an expiration date of 11/25. When the kit was opened, the inhaler inside was labeled with an expiration date of 11/30/25. An administrative nurse confirmed staff do not count the Ativan in the medication refrigerator unless a dose is removed and agreed it should be counted routinely. The nurse also confirmed the Lupin inhaler in the emergency kit was outdated and pharmacy should have replaced it.
