Citation history
Health deficiencies cited at Madison Health And Rehabilitation during CMS and state inspections, most recent first.
Surveyors identified multiple failures in food storage and sanitation, including undated bulk bins of dry ingredients, an opened and undated bag of sandwich rolls, and numerous yogurt containers stored past their use-by date alongside discolored lettuce in the walk-in cooler. The ice machine baffle was observed with pink and brown substances across its surface. The Dietary Manager reported that cooks were responsible for labeling and dating food and dish aides for discarding yogurt by its use-by date, and acknowledged that lettuce should have been discarded before spoilage. The Administrator stated that dietary staff were responsible for cleaning the ice machine and confirmed her expectation that all opened food be labeled, dated, and expired or spoiled items discarded.
The facility did not ensure that residents were protected from all forms of abuse and neglect, including physical, mental, and sexual abuse, as well as physical punishment, by any person.
The facility failed to remove expired deli meat from a kitchen refrigerator, as observed during a survey. The Dietary Manager acknowledged the oversight, stating that opened food should be discarded after 7 days. The morning cook is responsible for checking for expired food, but the deli meat was overlooked. The Administrator confirmed the expired meat would not have been served to residents.
Improper Food Storage, Labeling, and Unsanitary Ice Machine
Penalty
Summary
The deficiency involves failure to properly label, date, store, and discard food items and to maintain a clean and sanitary ice machine. During an initial kitchen observation, surveyors found undated bulk bins of sugar, flour, and cornmeal, as well as an opened and undated bag of submarine sandwich rolls in the dry storage room. In the walk-in cooler, surveyors observed multiple containers of yogurt past their use-by date and a box of lettuce with brown discoloration. The Dietary Manager stated that first and second shift cooks were responsible for labeling and dating food, and dish aides were responsible for checking yogurt use-by dates and discarding them on or before those dates. She also reported that she had asked second shift staff to discard the lettuce two days earlier, and acknowledged it should have been used or discarded before showing signs of spoilage. Surveyors also observed the facility’s ice machine and noted pink and brown substances scattered across the entire baffle inside the machine. The Dietary Manager initially indicated that Maintenance was responsible for cleaning the inside of the ice machine but stated she would expect the machine to be clean. The Administrator later clarified that dietary staff were responsible for cleaning the ice machine itself, while Maintenance was responsible for cleaning the filter. The Administrator stated her expectation that all opened food be labeled and dated and that expired or spoiled food be discarded, and later attributed the presence of expired and undated food to dietary staff feeling overwhelmed during a recent ice storm when some staff had to remain in the facility.
Failure to Protect Residents from Abuse and Neglect
Penalty
Summary
A deficiency was identified regarding the facility's failure to protect each resident from all types of abuse, including physical, mental, sexual abuse, physical punishment, and neglect by any individual. The report notes that residents were not adequately safeguarded from these forms of mistreatment, indicating lapses in the facility's protective measures and oversight.
Expired Food Found in Kitchen Refrigerator
Penalty
Summary
The facility failed to remove expired food from one of its kitchen refrigerators, specifically the walk-in refrigerator. During an observation, a resealable plastic bag containing deli meat was found with a date indicating it was past the 7-day storage limit. The Dietary Manager (DM) acknowledged that opened and stored food should be discarded after 7 days and immediately removed the expired deli meat. The DM explained that the morning cook is responsible for checking the refrigerator daily for expired food, but the deli meat was overlooked during the morning check. The Administrator confirmed that the outdated deli meat would not have been served to residents and was uncertain whether the meat was misdated or simply overlooked, reiterating the facility's policy of disposing of opened food items after 7 days.
