However, the quarterly MDS completed by the MDS Coordinator did not reflect that the resident received insulin injections or hypoglycemic medication. In interviews, the MDS Coordinator acknowledged overlooking and incorrectly coding these medications, and both the corporate MDS coordinator and the DON confirmed that the resident had been receiving daily insulin and that the MDS should have accurately captured relevant medications.

Inaccurate MDS Coding for Insulin and Hypoglycemic Medication

Review of the February 2026 Medication Administration Record (MAR) showed the resident received daily insulin injections as ordered. However, the resident’s quarterly MDS dated 02/20/26 did not indicate that insulin injections or hypoglycemic medications had been received. During interviews, the MDS Coordinator confirmed she completed the 02/20/26 MDS for this resident, acknowledged that the resident received insulin daily, and stated that the MDS should have included both insulin injections and the use of hypoglycemic medication but these were overlooked and coded incorrectly. The Corporate MDS Coordinator stated that all MDS assessments should be coded correctly for relevant medications. The Director of Nursing also confirmed that the resident had received daily insulin injections and that the MDS should be coded correctly for relevant medications.
Failure to Maintain Resident Room Window Screens in Good Repair

Surveyors observed that in one resident room on two separate days, the window screen’s metal frame was out of the track at the bottom of the window frame, leaving the screen unsecured and allowing the possibility for flying insects to enter if the window was opened. A subsequent exterior walking tour of the building with the Maintenance Director identified 18 rooms with window screen issues, including bent frames out of the tracks, missing screens, and multiple tears of varying sizes in the screens. Specific observations included rooms with no window screens in place, screens with tears ranging from approximately 1 inch by 3 inches to 4 inches by 3 inches, and screens with metal frames out of the metal tracks. One resident interviewed reported no concerns with the window screen and stated he had not opened his window since admission. The Maintenance Director, employed for about two and a half months, stated he had not yet established a good monitoring program for windows and was only aware of two screens needing replacement, both in his office. He indicated that staff could submit work orders for needed repairs but reported he had not received any work orders for screen repairs. The Administrator stated she expected staff to alert maintenance to issues and complete work orders, and reported she was unaware that so many screens required repair, noting that a previous Maintenance Director had assessed and repaired damaged screens in early fall of 2025.
