The deficiency was identified during review of interviews, observation, record review, facility investigation materials, and facility policy review. The event was determined to be an Immediate Jeopardy because the residents exited the facility unsupervised and staff did not know they were outside until they were alerted by others. On 5/18/26 at about 5:41 PM, a transportation service employee who was not employed by the facility exited through the front door using his badge for access. Resident #1 and Resident #2 followed him outside before the door closed. Video surveillance and the facility investigation showed that the residents remained on the facility grounds without staff awareness or supervision until about 5:52 PM, when a dietary aide saw them in the parking lot and a family member also called the facility to report them outside. Staff then responded and brought the residents back inside. Resident #1 was admitted with diagnoses including unspecified severe dementia with agitation and Alzheimer's disease, and had a BIMS score of 3, indicating severe cognitive impairment. Resident #2 was admitted with a diagnosis of dementia and had a BIMS score of 5, also indicating severe cognitive impairment. The facility investigation and staff interviews confirmed that both residents were ambulatory with walkers, were outside unattended, and were not known by staff to have exited the building until they were observed by others. The facility policy defined elopement as leaving the premises or a safe area without authorization or supervision, and the incident involved two residents who were not under staff supervision when they left the building.
