During interviews, LPN-B stated she was offered vaccinations every fall but they were no longer required for employment, that she had become very sick after her last COVID-19 vaccine, and that she did not recall receiving workplace education about COVID-19 vaccination. LA-A stated she received vaccine education and vaccines during a physician visit and did not recall receiving education at the facility. NA-A stated she received COVID-19 vaccine education in 2020 and had her initial series at the clinic, but did not receive a booster this year. LPN-A stated she had not received a booster this year, had a medical release form signed by her physician in 2020, had not received vaccine education at the facility, and had not provided a declination or copy of her medical exemption for facility retention. NA-B stated he was recently hired, received vaccine education during new hire orientation, and had already received his initial COVID-19 series. RN-A stated staff vaccinations were done through the clinic and that she retained no documentation at the facility, while the DON and administrator stated staff were expected to be educated and documentation should be retained.
