The resident’s care plan addressed self-administration of insulin via pump, but it did not include documentation for other medications that were present in the room, including triamcinolone acetonide cream for rash/itch and acetic acid otic solution for ear wax. The resident’s SAM form dated 4/1/25 identified the resident as safe to administer insulin from the pump, but did not identify the resident as safe to self-administer topical medication or ear drops. During interview, the resident stated she used the triamcinolone cream for rashes on her hands and used the ear drops for dry ears. Surveyors observed the resident’s triamcinolone cream in the pocket of her recliner and a bottle of acetic acid ear drops on the table next to the recliner while the resident was in the dining room. Staff interviews confirmed the resident self-administered insulin via pump, but the RN, DON, and consultant pharmacist all confirmed the topical cream and ear drops were not addressed on the SAM form and were not documented as approved for self-administration. The facility policy required a completed SAM form, a physician order specific to each medication to be self-administered, and documentation of which medications the resident was self-administering and where they were kept.
