On another occasion, during a medication pass observation on the 200 B medication cart, an Incruse inhaler and a fluticasone inhaler were observed without a date opened or a resident identifier. Two opened insulin pens were also found without an opened date. Further observations on the 200 A medication cart revealed a Lantus insulin pen and a Lispro insulin pen for the same resident without a date opened. A Humalog insulin pen and a Lantus insulin for a second resident were also without a resident identifier and an opened date. Two albuterol inhalers for a third resident were not dated when opened. The Director of Health Services acknowledged that staff should date items on the actual vial and noted that the pharmacy sometimes does not provide patient labels for medications such as inhalers and insulin pens. The facility's policy requires that when the original seal of a manufacturer's container or vial is broken, it should be dated, and a date opened sticker should be placed on the medication.

The infection control nurse and the Director of Health Services confirmed the necessity of using gloves to pick up pills, performing hand hygiene before and after glove use, and cleaning the blood pressure cuff after each use. The facility's policy on medication administration, revised in November 2018, requires adherence to good hand hygiene practices, including before handling medications, after resident contact, and at regular intervals during medication passes. The observations revealed non-compliance with these guidelines, contributing to the deficiency in infection control practices.
