The facility failed to store and prepare food in accordance with professional standards of food service safety. During the initial kitchen tour on 12/08/2025, surveyors and the Certified Dietary Manager observed multiple expired food items in the walk-in refrigerator, including black beans, maraschino cherries, walnut topping, cream of crab soup, corned beef, Kalamata olive oil, Dannon yogurt, and goat cheese. Two cheese balls and one cheese log were also observed with no expiration date. During an interview at that time, the CDM stated the expired foods would be discarded and that all food items were expected to be labeled with an open or prepared date and expiration date. During a continued kitchen tour on 12/10/2025, surveyors and the Director of Dietary observed food and debris on the metal shelf where bowls were stored. They also observed a brown paper bag in the kitchen refrigerator containing two white disposable food containers and two bags of chips; the bag and containers were not labeled or dated. The Director of Dietary stated the containers were for residents but could not identify which residents or when the food had been placed in the refrigerator. After asking a kitchen employee, the Director of Dietary was told the containers were for two residents and had been placed in the refrigerator that day. On 12/11/2025, surveyors and a dietary aide observed additional expired items in the satellite kitchen refrigerator, including apple juice and caramel sauce.
