The facility failed to ensure staff practiced infection prevention and control measures in the laundry area. During observation, Laundry Aid #15 was in the washer room, which was identified as the dirty area, and then came to the clean side without washing or sanitizing her hands. She took a pile of clean towels from the folding table, placed them on a cart, and continued folding laundry. When interviewed at the time of the observation, she confirmed that she had not washed or sanitized her hands when leaving the dirty area and continued folding clean laundry after the concern was brought to her attention. Additional observations in the laundry area showed conditions that did not maintain separation between dirty and clean items. In the receiving area, boxes were stacked on the floor near the doorway, refill bottles of hand sanitizer and packages of tablecloths were stored above the dirty laundry bins, and the doorway between the receiving area and washer room was open with multiple blankets and a shower curtain hanging on it. Laundry Aid #15 identified those items as clean and removed them from the door to take them to the clean side to fold and place on a clean cart without washing her hands. Foam cushions without covers, a shoe, mop heads, mops, buckets, linen carts with covers draped open, resident clothing carts draped open, and clean linens stored in a closet with boxes on the floor were also observed in the laundry area.

The shelf holding the trashcans also had a dried brownish substance on it. During interview, the Housekeeping Supervisor, Project Manager, and Environmental Services Director stated there had been a juice machine leak on the floor above the area and that they were aware of the damage and the need to clean the area, but no rationale was provided for why the area had not been cleaned and repaired.
