A resident with severe cognitive impairment, dementia, anxiety, diabetes, and heart failure was assessed as an elopement risk, required constant supervision, and had a wander guard bracelet identified in the care plan and elopement assessment. The facility’s policy stated that residents with wandering behavior were to have a wander guard placed to audibly alert staff of attempts to exit the facility, and that placement and function were to be checked by nursing as ordered. The resident’s behavior care plan directed staff to redirect the resident, and the fall care plan stated the resident required continual supervision and staff assistance at times. On the evening of the incident, staff brought the resident’s dinner tray to the room and found the resident missing. Staff searched the facility, could not locate the resident, and called 911. The resident was later found by police on a neighbor’s front porch in the surrounding neighborhood and returned to the facility unharmed. Staff interviews indicated no one saw the resident leave and no alarm sounded. A CNA said the exit door near the resident’s room opened without entering the code and did not alarm when tested after the elopement. The Maintenance Director said he did not regularly check the wander guard system and could not provide documentation that it had been checked, and nursing staff said the wander guard had not alarmed when the resident was near an exit door.
