Health deficiencies cited at Newbridge On The Charles Skilled Nursing Facility during CMS and state inspections, most recent first.
An LPN left medication cups unattended and unsecured on a med cart during passes for two residents while stepping away to a nurses’ station to obtain additional meds from the ADM. The nurse prepared multiple meds for one resident and later for another resident, but in both instances the pills were left out of view in the hallway instead of remaining with the nurse during administration; the DON stated this should not occur.

Summary
The facility failed to ensure drugs and biologicals were stored in accordance with accepted professional principles when medications were left unattended and unsecured during medication passes. The deficiency involved Resident #24 and Resident #62, and the report states that medications were not kept in view of the nurse while the nurse moved away from the medication cart and left the cups of pills on top of the cart in the hallway. For Resident #24, Nurse #1 prepared multiple medications, left the cup on the medication cart, entered the resident’s room, then walked to the nurses’ station to remove Tylenol from the ADM while the medications remained unsecured and out of view. The nurse repeated this pattern before administering the medications. For Resident #62, Nurse #1 removed several medications from the ADM, placed them in a cup on the cart, entered the resident’s room to scan the name band, then left the cup unattended on the cart while she went to the nurses’ station to obtain Oxycodone from the ADM. Nurse #1 stated she should not have left the pills unattended, and the DON stated medications should not be left unsecured and out of the nurse’s view, especially while obtaining additional medication.
