A medication storage room on one unit was left unlocked and unattended when a Nurse allowed Maintenance Personnel into the room and then walked away. The Maintenance Personnel propped the door open and left it unsecured on two occasions, while residents and other staff were nearby. Surveyors later observed medications, OTC meds, syringes, and resident bubble packs accessible in the room; the DON stated only Nurses were authorized access and that a LPN/LVN or RN should supervise Maintenance Personnel.

Summary
The facility failed to ensure that the medication storage room on the [NAME] Unit was locked when unattended and was under direct supervision of authorized personnel when unlocked. Surveyors observed a Nurse unlock the medication storage room door for Maintenance Personnel and then walk away, leaving the Maintenance Personnel unsupervised in the room with the door unlocked. The Maintenance Personnel later used a piece of wood to prop the door open and left the area, leaving the medication storage room unsecured. Surveyors also observed residents and other staff walking around the nurse's station area while no authorized personnel were present to supervise the unlocked room. On a later observation, surveyors found medications accessible on the storage room shelves, multiple over-the-counter medications in a black container on the counter, as well as syringes and prescribed bubble pack medications for specific residents. During interview, a Nurse stated that only insulin and other narcotics were locked within another compartment in the room. The DON stated that Nurses were the only staff authorized access to the medication storage room, that a Licensed Nurse was to be present if Maintenance Personnel were in the room, and that the room should not be left propped open and unsupervised.
