The facility's policy on abuse prevention and investigation was not effectively implemented, as evidenced by the lack of adequate interventions to prevent Resident #180's aggressive behavior. Despite being monitored every 30 minutes, no additional measures were put in place to address the risk of resident-to-resident abuse. The incident was reported to the Director of Nursing, who confirmed the occurrence of physical abuse, but the facility's response was insufficient to prevent the incident from happening.

Despite being cognitively intact and not refusing care, the resident did not receive the prescribed intervention, as confirmed by the absence of documentation of refusal in the progress notes.
