Provide safe and appropriate respiratory care for a resident when needed was not ensured for R144. R144 was admitted with diagnoses including COPD, CKD, and ischemic cardiomyopathy, and the MDS indicated the resident was cognitively intact with a BIMS score of 15 out of 15. The care plan included an intervention to administer oxygen therapy as ordered by the physician, and the physician order dated 09/04/2025 specified oxygen at four LPM via nasal cannula. Observations on 05/04/2026, 05/05/2026, and 05/06/2026 showed R144 receiving oxygen at five LPM via nasal cannula instead of the ordered four LPM. During these observations, the oxygen tubing was not labeled or dated, and the humidification bottle was also not labeled or dated; on 05/06/2026 the humidification bottle was empty. In interview, LPN3 stated nursing staff were responsible for checking and changing oxygen humidification and tubing weekly or as needed and for verifying oxygen requirements by reviewing the MAR or TAR, but she stated R144's oxygen should be set at three liters. Review of the MAR/TAR showed an active order for oxygen at four LPM, and LPN3 then verified the regulator was set at five LPM. The UM stated nursing staff were to check residents' oxygen, including tubing and humidification, every shift and were expected to follow physician orders concerning medication administration.
