A facility failed to provide a safe, clean, comfortable, and homelike environment for two cognitively intact residents sharing a room. Observations found dirty sheets, a strong urine odor, flies and gnats throughout the room, opened soda cans, old food in a refrigerator, and empty milk cartons on the sink. One resident said staff removed belongings without permission during cleaning, and the other said he asked for a clean room free from flies, gnats, and urine odor. The DON and ED both stated the room was not homelike.
A resident with type 2 DM, cerebrovascular disease, and a recurrent inguinal hernia had a scheduled CT scan cancelled because the facility had no transportation available. Although the resident was ambulatory and the facility had insured drivers available, staff stated the main driver called off and the appointment was still missed. The DON stated the resident was responsible for facility transportation to outside appointments, and the resident reported frustration with repeated cancelled appointments.

On observation, their shared room was malodorous and smelled of urine, with gnats and flies present in the room. The room contained dirty bed sheets, flies on the sheets, a heavy presence of gnats, a clear tote with 13 opened soda cans, a personal refrigerator with a foul odor and old food, and empty milk cartons on the sink with gnats landing and flying around them. A later observation again found dirty sheets, flies around R30’s genital area, a heavy presence of gnats, and a strong urine odor. R30 stated he had flies, gnats, and an odor in the room and said staff had thrown away or removed his belongings without his permission when cleaning. R45 stated he had asked staff for a clean room free from flies, gnats, and urine odor, and said the conditions were not homelike. Housekeeping, the DON, and the ED all stated the room had a strong urine odor, molded food, flies, and gnats, and the ED stated she did not consider the room homelike.
