Observations revealed that food temperatures were not consistently recorded on the weekly menu spreadsheets, with only partial documentation for both first and second shifts. Additionally, sanitizer test strips were not consistently attached or recorded for all days, and the parts per million (PPM) concentration of the sanitizer was not documented for the entire month reviewed. Staff interviews confirmed that food temperatures and sanitizer concentrations were not always properly recorded, and that the cleaning cloth was not kept in the sanitizer bucket between uses as required by policy. Further observations showed that the cook used a cleaning cloth to wipe surfaces without placing it in a sanitizer bucket after each use, and no sanitizer bucket was visible during food preparation. When the sanitizer buckets were checked, neither contained sanitizer at the correct strength, with one bucket containing only water and the other containing soapy water. Staff were unclear about which bucket contained sanitizer, and the test strips did not indicate the presence of sanitizer. The facility's own policies and FDA guidelines require proper preparation and monitoring of sanitizing solutions and consistent documentation of food temperatures, which were not followed in these instances.

On the second floor, a box of Fluzone High Dose influenza vaccine with an expiration date of June 2024 was found in the medication refrigerator, with one remaining syringe. Additionally, a Proair inhaler for a resident was found expired since January 2024, and an unopened Levemir Flexpen was improperly stored in the medication cart instead of the refrigerator. Other expired items included nystatin and triamcinolone cream, Biofreeze, and sugar-free cough drops, all of which were past their expiration dates. Interviews with a Certified Medication Technician (CMT) revealed that night shift nurses were responsible for checking expired medications, but the expired items were not identified or removed. On the third floor, an expired Insulin lispro pen and a fluticasone nasal spray that had exceeded the recommended number of actuations were found in the medication cart. The Director of Nursing and the Administrator both acknowledged that the night shift nurses were expected to remove expired medications, but this task was not completed. The Director of Nursing noted that the night shift nurses either did not perform their duties or failed to notice the expired medications, while the Administrator emphasized that expired medications should not be used.
