Short Summary

The facility failed to remove expired medications from storage, including a Fluzone vaccine, Proair inhaler, Levemir Flexpen, and other items. Observations revealed expired medications in two medication carts and a refrigerator. Interviews indicated that night shift nurses were responsible for checking and removing expired medications, but this was not done. The DON and Administrator acknowledged the oversight.
