Dietary staff confirmed the cookware was not sanitary and needed cleaning or replacement. The facility policy required food-contact cookware to be maintained in a sanitary and safe condition, including discard criteria for pitting.
A resident with hemiplegia, coordination deficits, and Parkinson’s disease had a care plan and MD order for a left hand/wrist/finger splint during sleeping hours, but the MAR/TAR repeatedly showed the splint as applied and removed even when it was not actually on. During observation, the resident had no splint on and stated he often did not wear it, while CNAs and an LN confirmed refusals should be documented and that the record should accurately reflect the care provided.

The facility failed to prepare food under sanitary conditions in the main kitchen, where all resident meals were prepared before distribution to two satellite kitchens, including the east kitchenette serving long-term care and skilled residents. During a kitchen tour, surveyors observed three pots with black and brown buildup on the outside and inside in areas that would contact food, and 24 sheet pans with baked-on brown substance inside, outside, and along the outer rims. The sheet pans were stacked one on top of another, and Dietary Staff CC verified that the pots and sheet pans were not sanitary and needed cleaning and/or replacement. On a follow-up kitchen tour, surveyors observed a four-quart and an eight-quart aluminum pot with pitted insides and discolored bottoms in areas that would directly contact food during preparation. Dietary Staff CC confirmed that the inside surfaces of both pots were not sanitary and needed replacement. The facility policy dated 01/06/2025 stated that food-contact cookware must be maintained in a sanitary and safe condition and defined removal from use, deep cleaning, discard criteria including pitting, and replacement procedures when items were beyond restoration.

The resident had diagnoses including hemiplegia and hemiparesis following a cerebral infarction affecting the left non-dominant side, other lack of coordination, and Parkinson’s disease with dyskinesia. His MDS assessments documented intact cognition with a BIMS score of 15, impairment on one side, and need for assistance with ADLs, including oral and personal hygiene and toileting hygiene. The resident’s care plan directed staff to assist him with wearing a splint on his left hand and middle finger during sleeping hours and to remove it when he was up. A physician order later directed staff to assist him with wearing a splint on his left hand, wrist, and finger during sleeping hours and remove it when up. However, the MAR/TAR from 12/01/25 through 12/17/25 documented that the arm splint had been applied at 8:00 PM and removed at 8:00 AM each day, with no refusals documented. During observation, the resident was in bed with his left fingers contracted and swollen and did not have a brace or splint on his left hand. The resident stated he did not like calling staff to assist him and said he had just awakened from a nap; he also stated he never wore the arm brace at night because it made it difficult to do things. CNAs stated that the resident sometimes refused the brace, that refusals should be reported and documented, and that he did not wear the brace during the day. An LN and an administrative nurse stated that CNAs completed the task but nurses documented it on the TAR, and that refusals should be charted and reflected accurately in the clinical record. The facility policy stated that application and removal of splint/brace/slings were to be completed on the TAR.
