Food was not stored, prepared, distributed, and served under sanitary conditions in the facility kitchen. During the kitchen tour, surveyors observed an opened 3-pound bag of shredded lettuce and an opened 3-pound bag of slaw in the refrigerator without opened dates, along with six unlabeled one-ounce ketchup containers and a one-gallon container of Italian dressing that was about half full and not labeled with the date opened. The kitchen also had an unsanitizable countertop with exposed wood at the corner, a metal entrance door with missing paint around the handle, loose caulking and brown staining at the handwashing sink where it met the wall, ten large cookie sheets and one small cookie sheet with baked-on brown grime, and grease buildup on the outer ridge of the gas stove hood. Further observation showed the oven interior and racks had baked-on grime and grease throughout, and dietary staff stated the oven had to remain on at 500 degrees Fahrenheit starting at 5:30 AM to cook meals and stayed on all day, leaving no safe time to clean it after staff left at 6:00 PM. Nursing staff were also observed entering the food preparation area without washing their hands, handling plated food, and placing condiment packets directly on top of the food before delivering plates to residents. One CNA touched a resident’s back and returned to the kitchen without hand hygiene, then repeated the same process. Dietary staff confirmed the condiment packets were placed directly on the food and acknowledged this was not sanitary.
