An LPN was seen mishandling PPE and neglecting hand hygiene, while a housekeeper entered a COVID-19 room without proper PPE. A resident left a COVID-19 room without a mask and was not informed of isolation precautions, contrary to facility policy.

The facility failed to ensure food was served in a sanitary manner during 2 kitchen observations. During an observation of the kitchen, dirt and debris were seen on the floor behind the stove, fryer, convection oven, and steamer, as well as under stainless steel tables and racks. Debris and grease were observed on the top and sides of the stove and on the sides of the fryer. Dirt and debris were also seen behind the ice machine, steam table, and plate warmer, and on surfaces in the dry pantry, including the floor and under the racks. When the same areas were observed again later, the same conditions were still present. The Dietary Manager stated the kitchen is cleaned daily and floors are usually cleaned at night, but this did not get done the prior night due to short staffing. The Administrator provided the kitchen cleanliness policy, which stated that a clean, sanitary, and safe kitchen environment will be maintained at all times and that kitchen areas, including food preparation surfaces, equipment, storage areas, walls, and floors, must be cleaned regularly and kept free of debris, spills, and potential contaminants.

The facility's policy requires that PPE be doffed and discarded before leaving a room and that residents' movement outside their rooms be limited to medically necessary purposes, with masks worn during any essential movement.
