The August 2025 TAR showed the resident received dialysis on 8/1/25, 8/4/25, 8/6/25, 8/8/25, 8/11/25, and 8/13/25, but the clinical record lacked a Pre-Dialysis Assessment Observation dated 8/4/25 and lacked Post-Dialysis Assessment Observations dated 8/1/25, 8/4/25, and 8/8/25. The record also lacked documentation that the dialysis site was monitored daily for signs and symptoms of infection and bleeding, and Resident 7’s progress notes did not document that the dialysis center was notified to request a communication form or run report. Staff interviews indicated the Pre-Dialysis Assessment Observation went with the resident to dialysis, the dialysis center did not always send a communication form back, and the facility completed the Post-Dialysis Assessment after the resident returned. The DON stated she needed to call the dialysis center to obtain the August 2025 dialysis communication or run report, and the run reports were not in the clinical record. The facility policy required completion of pre- and post-dialysis assessments, but it did not address the dialysis center sending a communication report back to the facility.
