Food safety and sanitation were not maintained in the kitchen freezer when surveyors observed expired and improperly labeled food items. On 05/01/2026 at 8:10 AM, the first-floor freezer contained one pack of prepacked sausages and twelve beef patties with expiration dates of April 2022 and February 2025, along with nine prepacked ziplock bags of chicken, seven prepacked ziplock bags of fish, a half-bag of shrimp, and seven racks of bread products that had no labels or dates showing an open date or end date. Stock/Utility Staff GG confirmed that the expired items should have been discarded and that all food items should have been labeled with an open date and an end date. The CDM later confirmed the expired turkey, sausage, and beef patties should have been discarded and that the chicken, fish, shrimp, and bread products should have been labeled and dated. The RD also confirmed that staff should have labeled all food items with an open date and an end date and discarded all expired food items.

The facility failed to ensure the medication error rate remained below 5% during medication administration observations, with 3 errors identified out of 25 medication opportunities, resulting in a 12% error rate for two residents reviewed. Facility policy required the six rights of medication administration, including right documentation, and required that medications be crushed only as ordered and not crushed when labeled do not crush. The policy also required the MAR to be signed after medications were administered. During observation in Farm House, an LPN did not administer clotrimazole-betamethasone cream and hydrocortisone cream to one resident but documented on the MAR that both creams had been administered as ordered. The LPN and DON later confirmed the MAR was signed in error. During another observation in Meadow House, an RN crushed and administered enteric coated aspirin 325 mg to another resident with an order for one tablet by mouth daily for PVD. The RN stated medications with coating should not be crushed, and the LPN and ADON also confirmed that enteric coated medications should not be crushed.
