The facility failed to store stacked steam table pans free from wet nesting. During an observation of the dish room, a store rack contained stacks of steam table pans, and when a stack of three small square steam table pans was pulled apart, moisture was observed inside the top and middle pans. The Certified Dietary Manager later verified that the two pans had been stacked and stored with moisture inside, and stated that dietary staff were expected to stack and store pans only after they were dry inside. The facility also failed to properly demonstrate use of the three-compartment sink when washing and sanitizing food processor parts. A dietary aide washed the food processor bowl and lid, rinsed them, placed each item in the sanitizing solution for only a few seconds, and then placed them on a drying rack. The aide then re-washed and sanitized the items and dried the inside of the bowl and lid with paper towels. The CDM confirmed that the items were not left in the sanitizing solution for the required one minute and stated that staff should air-dry sanitized dishware rather than use paper towels. The aide acknowledged not following the posted guidance above the sink and said he did not keep the items in the sanitizer for one minute because he forgot.

Review of the resident’s quarterly MDS assessment showed that the resident received a diuretic during the seven-day lookback period, and review of the care plan showed that no care plan had been developed for the resident’s use of diuretics. The facility policy titled Patient's Plan of Care stated that each resident should have a comprehensive, person-centered care plan addressing medical, physical, mental, and psychosocial needs and that the care plan should be updated as clinical assessments changed over time. During interview, the MDS Coordinator stated she was responsible for developing and updating care plans based on diagnoses, changes in care, and MDS assessments, and acknowledged that the care plan for diuretic use had not been developed due to an oversight. The DON stated she expected the MDS Coordinator to develop and update care plans for every resident in the facility.
