Dietary staff failed to follow facility puree recipes for vegetable blend and mashed potatoes for eight residents on a puree diet. Observations showed the CDM and Assistant Kitchen Manager altered the preparation by adding hot water, using broth instead of the recipe directions, and omitting ingredients such as skim milk, while the Division Dietician stated that puree recipes should be followed.
Resident Trust Fund Statements Not Provided: A resident with little to no cognitive impairment did not receive quarterly trust fund statements as required. The resident and his daughter both stated they had not received account balance information, while staff said the resident had not signed the statements because he disagreed with the contents, but no documentation showed a refusal to sign.
Infection control practices were not followed for three residents during blood sugar checks and medication administration. An RN returned a glucometer to the med cart without sanitizing it after use on one resident, then used a plastic container and bedside table for another resident without a protective barrier or sanitizing the surfaces, and an LPN administered meds via gastric tube to a resident without sanitizing the bedside table before or after placing supplies on it.

The facility failed to ensure dietary staff followed the facility-provided recipes when preparing pureed meals for eight of eight residents who received a puree diet. The recipe for Vegetable Blend Capri Pureed Thick instructed staff to drain the vegetables, process them until smooth, and then add food thickener, but during observation the Assistant Kitchen Manager measured 4 and a half scoops of carrots, pureed them, added hot water to the bowl, pureed them again, and then added a scoop of thickener before placing the food in a warmer. She stated she wanted to make sure the carrots were pureed completely. The recipe for Mashed Potato Insta Prep (Flakes) instructed staff to use boiling water, skim milk, potato granules, unsalted butter, and salt, but during observation the CDM prepared the potatoes by placing 4 cups of hot water in a metal container, adding salt, pouring in potato flakes, and mixing with a whisk before covering and placing the food in a warmer. During interview, the CDM stated that when liquid needed to be added to the vegetable puree, she would add vegetable broth, and she further stated she did not need to add broth to the vegetables. When asked how much potato flakes were added, she stated maybe 2 cups, and when asked if the recipe included milk, she stated she should have followed the recipe and added milk when preparing the potatoes. The Division Dietician stated that when pureeing food, the recipe should be followed.
Resident Trust Fund Statements Not Provided

The facility failed to provide quarterly resident trust fund statements to one resident, R68, whose personal money was deposited with the nursing home. The facility policy titled, Patient Trust Fund, stated quarterly statements should be provided in writing within 30 days after the end of the quarter and that the patient should sign the statement acknowledging accuracy, or if unable to sign, a copy should be mailed to the responsible party for signature. Review of the trust fund records for R68 showed a statement dated 1/30/2025 with the resident's signature, but the statements dated 4/29/2025 and 7/21/2025 did not have signatures. R68 had a BIMS score of 15 on the annual MDS, indicating little to no cognitive impairment, and no behaviors were documented. During an interview, R68 stated he had not been able to receive his quarterly trust fund statements and wanted to know the account balance, but did not receive it. He also stated that when his daughter sent a check, it was given to the office, but he was unsure whether it went where it should because he had not received follow-up. R68's daughter stated she was unsure of the account balance and had not received any financial statements from the facility. Staff stated the resident trust statements were provided every three months and that the resident signed a form acknowledging receipt, but also stated R68 had not signed because he disagreed with the contents; however, no documentation was available showing that R68 refused to sign the statements.
Infection Control Practices Not Followed During Resident Care

The facility failed to ensure infection control practices were followed for three sampled residents, R19, R74, and R10, during medication administration and blood sugar monitoring. The facility policy titled, Infection Prevention Plan, stated that infection prevention practices are intended to promote a healthy and safe environment and reduce the risk of infections, and that epidemiological principles and an interdisciplinary approach shall be used to focus on surveillance, prevention, and control of infections. On 7/23/2025 at 9:50 AM, RN CC performed a fingerstick blood sugar test on R19 with a glucometer and returned the glucometer to the medication cart without sanitizing it. Later that day at 11:55 AM, RN CC transported an insulin pen, glucometer, alcohol wipes, and a cotton ball in a plastic rectangular container to R74's room without lining the container with a protective barrier. She placed the container on the resident's bedside table without a protective barrier or sanitizing the table, performed a fingerstick blood sugar test, and then placed the glucometer, blood-stained cotton ball, and alcohol wipe on the bedside table. She returned the supplies to the container and took the glucometer back to the medication cart after sanitizing it, but did not sanitize the container or place a barrier in it. At 3:30 PM, LPN JJ administered medications to R10 via gastric tube, placed a barrier on the bedside table, and set supplies on it, but did not sanitize the table before or after use; she stated she presumed housekeeping cleaned the table frequently.
