The MDS also indicated the resident receives an anticoagulant. Review of the resident’s physician orders showed an order dated 4/3/2025 for apixaban 5 mg by mouth twice a day. However, the resident’s comprehensive care plan, initiated on 3/12/2025, did not display the use of an anticoagulant. During interviews, an LPN stated an anticoagulant should be on the care plan to help communicate the resident’s care needs, the MDSC stated it should be on the care plan so staff know the resident’s needs, and the DON stated it should have been on the care plan to let staff know the anticoagulant was being used and how to monitor for it.
