An observation on 6/18/24 revealed that the resident had a large adhesive dressing on his left leg, dated 6/14/24, which did not align with the physician's order. The order, dated 6/12/24, instructed the nursing staff to cleanse the skin tear with normal saline, apply antibiotic ointment, and cover it with a Band-Aid once a day, every other day until healed. However, the treatment administration record (TAR) indicated that the treatment was completed as ordered on 6/16/24, which was inconsistent with the dressing date observed. During an interview, RN A confirmed that she was responsible for the resident's skin treatment on 6/16/24 and had signed the TAR to indicate the treatment was completed. She acknowledged that the resident's wife took him out on that day, and he did not want the bandage changed before leaving. RN A admitted she should have documented that the treatment was not administered on 6/16/24. The facility's policy for Charting and Documentation requires that all services provided or any changes in the resident's condition be documented, including instances where the resident refuses treatment.
