During an observation of the resident in her room, a medication cup containing several pills was seen sitting on her overbed table, and the resident stated she thought she was asleep when the nurse brought the medications into her room. The nurse later stated she gave the cup of medications to the resident and did not observe her take the medications. A review of the resident’s electronic medical record showed a self-administration of medication observation form dated 2/7/26 stating the resident did not wish to self-administer medications, it was not appropriate for her to self-administer any medications, and she had a history of dementia. The record also showed no care plan or current physician order for self-administration of medications. The facility’s clinical guidelines stated that if a resident desires to participate in self-administration, the interdisciplinary team will assess competence, the nurse will interview the resident, obtain a physician’s order, educate the resident on reactions and side effects, and evaluate compliance.
