The DON stated only RNs or IV-certified LPNs could administer IV meds and acknowledged the IV administration could not be verified.

The facility failed to ensure food service items were handled with accepted food-handling practices during meal service from the main kitchen. During tray line observation, Dietary Aide A dropped the thermometer used to take food temperatures, gave it to the CDM, removed his gloves, obtained a new thermometer, and then placed a glove on his right hand. He wiped the new thermometer probe with an alcohol pad and continued taking food temperatures, but after cleaning the probe between food items, he touched the metal probe as he turned to take the temperature of the cream corn. He was stopped by the surveyor just before inserting the contaminated probe into the cream corn and then cleaned the thermometer again before continuing. During the same tray line observation, Dietary Aide B was observed preparing trays for meal service in front of the steam table and licking her fingers twice as she thumbed through the tray tickets. The tray tickets she touched after licking her thumb were then placed on resident meal trays. On interview, the CDM acknowledged the staff errors and stated staff should have been more aware of their actions. The facility policy on safe food handling, revised 4/30/25, instructed staff to wash hands before handling clean equipment and utensils, after touching hair or mouth, and before donning gloves for food-related tasks, and stated the food thermometer probe should be sanitized with an alcohol wipe before each use.

The facility failed to provide pharmaceutical services consistent with professional standards of practice for one resident receiving intravenous antibiotics. Resident #4 was admitted with diagnoses including metabolic encephalopathy, type 2 diabetes, sepsis due to E. coli, stage 3 chronic kidney disease, acute prostatitis, adult failure to thrive, and prostate abscess. The medical record showed a physician order for Piperacillin Sodium-Tazobactam Sodium 4.5 GM IV every 8 hours for urosepsis and prostatitis, and the resident also had a care plan for IV antibiotic therapy related to urosepsis with instructions for nurses to administer antibiotics as ordered. Review of the eMAR showed multiple scheduled IV antibiotic doses with blank documentation and no record that the medication was administered, including doses on 11/20/25, 11/21/25, 11/25/25, 11/27/25, 11/28/25, 12/1/25, 12/2/25, 12/4/25, and 12/9/25. The DON stated that only RNs or IV-certified LPNs could administer IV medications and said she assumed the RN gave the medication and forgot to document it, but acknowledged the eMAR did not reflect administration and that IV medication administration for the resident could not be verified.
