During a tour conducted with the facility's Administrator and Maintenance Director, it was observed that the East Side Shower Room had a significant build-up of dark matter on the walls, grout lines, and floors in all three shower stalls. Similarly, the [NAME] Side Shower Room had two out of three shower stalls with the same issue. In the East Side Soiled Utility Room, garbage cans were found without lids, and a laundry cart was uncovered with soiled linen not secured in a bag. Additionally, a large amount of garbage was found under the moveable bottom of the cart, and there was a significant build-up of ice in the freezer of the specimen refrigerator. In the [NAME] Side Soiled Utility Room, similar issues were noted with an uncovered laundry cart containing unsecured soiled linen and garbage. The laundry room's soiled area had uncovered garbage cans and laundry bins, with one bin's inner liner ripped and both bins containing dirt and debris. The dryer room had two out of three dryers with a large build-up of lint and foreign substances in the lint trap areas. An interview with a Laundry Aide revealed that lint traps were cleaned every two hours by laundry staff, and monthly by the maintenance department, as confirmed by the Maintenance Director.
