An active physician order dated 9/30/25 stated neutropenic precautions for the diagnosis of myelophthesis. The care plan included goals related to thrombocytopenia, pancytopenia, neutropenia, myelofibrosis, and myelophthisis, but it did not include interventions related to neutropenic precautions. On 2/04/2026 at 9:17 AM, Staff F, an RN and MDS coordinator, reviewed the care plan and confirmed that interventions should include neutropenic precautions and stated she would add it into the care plan.
Missing Tracheostomy Supplies at Bedside

During observations on 02/03/2026 at 09:00 AM, 12:45 PM, and 02:35 PM, and again on 02/04/2026 at 09:00 AM, the resident was in bed with the tracheostomy in place, but no extra tracheostomy inner cannula or obturator was observed at the bedside. The resident’s comprehensive care plan identified that the resident had a tracheostomy and was at increased risk for infection/complications, and it included an intervention to keep an extra trach tube and obturator at the bedside if the tube was coughed out. During interview on 02/04/2026 at 09:45 AM, an LPN stated that a spare tracheostomy inner cannula and obturator is usually at bedside, checked the room, and verified that the resident did not have the spare equipment, adding that residents with a tracheostomy should always have spare tracheostomy inner cannula and obturator at bedside.

The DON also stated staff did not wear a mask because the resident had a weakened immune system, and that if the resident left the room, the resident would wear a mask. The resident’s primary MD stated neutropenic precautions were protective measures and expected staff to wash hands and at minimum wear a mask when entering the room, with gown and gloves being very essential when providing care. A facility in-service dated 2/2/26 titled neutropenic precautions stated to use standard precautions and wear a mask when entering the room.
