Review of the TAR showed no documentation that this treatment was completed on December 27, 2025, January 1, 2026, or January 3, 2026. During interview and record review, the DON acknowledged the missing documentation and stated the task was supposed to have been completed and documented. The IP stated that cleansing the GT site and placing a new T-drain dressing daily was important for infection prevention. The facility policy for enteral feeding site care stated the purpose was to promote cleanliness and protect the site from irritation, breakdown, and infection.
