His care plan noted that he rolled his wheelchair near other residents in a seemingly intentional manner and into tight spaces without attending to circumstances, and that this behavior and teasing could trigger other residents and place them at risk for contact. The incident occurred while the resident was in the television room and approached the other resident in his wheelchair. A progress note stated the wheelchair contacted the other resident’s wheelchair before the resident struck him. During interview, a program leader stated the resident backed into the television room without looking, bumped the other resident, punched him on the lips, and tried to punch him again. The program leader also stated she was alone monitoring residents in two television rooms and that at least two program leaders should be in the television room to monitor residents. The facility also failed to protect another resident from physical abuse when a resident with schizophrenia and dementia struck him in the nasal area with closed hands in the television room, knocking off his glasses and causing an abrasion on the bridge of his nose. The injured resident had impaired cognition, used a walker, and was wearing headphones at the time, which limited his ability to hear the other resident. A progress note and staff interviews stated the resident became upset after the other resident did not move his walker and then hit him in the face. A program leader stated he was the only staff present and that at least two program leaders should be in the television room to monitor residents. The director of therapeutic recreation stated that three to four staff should be in the television rooms each shift to manage resident behaviors and the environment.
