PRN Oxycodone was administered multiple times to a resident with a right femur fracture and chronic medical conditions without documenting the pain scale required by the order. The MAR showed 19 administrations without a recorded pain rating, and both an LPN and the DON confirmed the pain level was not documented before the medication was given.
A resident with severe cognitive impairment had two bottles of nasal saline spray observed on the bedside table, including one in plain view, even though there was no order for the medication. Staff identified the spray as not prescribed for the resident and stated the resident should not have it in the room. The record also lacked an order and assessment for self-administration, although the care plan was updated to reflect the resident’s wish to self-administer the nasal spray.
Improper food storage and sanitation monitoring were observed in the kitchen. Open and exposed items were found in the refrigerator, freezer, and dry storage, including beans, vegetables, seasonings, rice, noodles, and bouillon, with food droppings and splatter also noted around the storage racks. Sanitation buckets tested below the required ppm on multiple checks, and refrigerated tray line and meal cart items were observed at elevated temperatures.
PRN Oxycodone Given Without Documented Pain Scale
Penalty
Summary
The facility failed to provide safe, appropriate pain management for a resident admitted with morbid obesity, Type II diabetes, and end-stage renal failure, who also had a displaced intertrochanteric fracture of the right femur and was identified in the care plan as having acute right hip pain. The care plan directed staff to administer analgesic medication as ordered, and the medication record showed orders for Oxycodone 5 mg every 6 hours as needed for pain rated 6 to 10, along with Tylenol 325 mg, two tablets every 4 hours as needed for pain rated 1 to 10. Review of the June 2021 MAR showed that the Oxycodone PRN order was administered 19 times without documenting the resident’s pain scale at the time of administration. During interview, an LPN stated that PRN pain medication requires a documented pain level to determine whether the order is appropriate and said she did not see a pain level recorded on the MAR. The DON stated that PRN pain medication should include a pain scale and non-medical intervention, and after reviewing the MAR and progress notes, stated that the pain level was not recorded prior to administering the Oxycodone and that the earlier order had been corrected on June 14, 2021.
Unsecured Nasal Spray at Bedside Without Self-Administration Assessment
Penalty
Summary
The facility failed to ensure medications were stored properly and failed to assess a resident for self-administration of medication for one resident with a diagnosis that included unspecified fracture of T9-T10 vertebra, subsequent encounter for fracture with routine healing, and generalized muscle weakness. The resident’s quarterly MDS showed a BIMS score of 09, indicating severe cognitive impairment, and no mood or behavior indicators. During the initial pool observation, two 1.5-oz bottles of Equate Nasal Saline Spray were seen in the resident’s room on the bedside table, including one bottle in full view and one unopened bottle in an empty Kleenex box. The resident stated staff were aware she had the medication. An LPN identified the bottles as nasal saline spray and stated the resident should not have them in her room and should not take medication that was not prescribed. The LPN reviewed the medication orders and found no order for the nasal saline spray, and stated there should also be an assessment to determine whether the resident was cognitively capable of administering her own medications. The care plan was updated to reflect the resident’s wish to self-administer normal saline nasal spray and included education and return demonstration, but the physician orders contained no self-administration order, and the record contained no self-administration assessment documentation. The DON stated the facility expectation was to follow policy, obtain an order, complete an assessment, and place the information in the care plan, and confirmed there were no orders or assessments for self-administration in the resident’s record.
Improper Food Storage and Sanitation Monitoring
Penalty
Summary
Food items were found improperly stored in the kitchen refrigerator, freezer, and dry storage during a walk-through with the Assistant Dietary Manager. In the refrigerator, a pan of pinto beans was open and exposed to air with ripped foil covering, and in the freezer several items were open or exposed, including broccoli cuts, garlic toast, ground rosemary, lemon pepper seasoning, dill weed, toasted sesame oil, and a box of long grain rice with spillage outside the packaging inside the box. In dry storage, food droppings were observed beneath the wire shelving and along the wall perimeter, along with an open box of lasagna noodles with broken pieces outside the packaging and an open 25 lb box of chicken bouillon exposed to air. Additional kitchen observations showed sanitation buckets testing below the required chemical level on two occasions, including the 3-compartment sink, meal prep sanitation bucket, and tray line sanitation bucket at 100 ppm when 200 ppm was required. On a later observation, dry storage food droppings remained beneath the wire racks, with oatmeal spillage near the base of the wheel and unidentified food splatter on the wall behind the racks. Meal cart and tray line refrigerated items were also observed at elevated temperatures, including pineapple chunks, mandarin oranges and pineapples, Crystal Light beverages, whole milk, and a health shake. The Dietary Manager stated the kitchen inspection process involved rounds by him and the Assistant Dietary Manager, that the dry storage area would be checked more frequently, and that sanitation solution changes would be increased to ensure proper ppm levels.
