A partially used box of frozen meat patties was stored without a received date or opened date, a box of garlic bread sticks was opened with the inner plastic bag left unsealed and the bread exposed, and a container of Italian seasoning had an opened date of 05/15/2025 and a use-by date of 05/15/2026. The Dietary Aide stated the particles were dust, the patties should have had dates, and the breadsticks should have been stored in a closed bag. During food temperature monitoring, a staff member tore open an alcohol wipe package with his teeth after putting it in his mouth, touched the package and wipe, wiped the thermometer, and then checked the temperature of pureed zucchini without performing hand hygiene after touching his face and the wipe package. The pureed food was served to six residents. The Dietary Manager stated the staff member should have stopped after putting the wipe in his mouth, the zucchini should have been discarded and remade, and the lack of hand hygiene and use of a wipe that had been in the staff member's mouth could expose residents to cross-contamination. Facility policy required clean, sanitized thermometers, food storage areas free of dirt, food items labeled with delivery dates, and handwashing after touching the face.

During a review of the facility's medication storage policy, it was noted that expired medications should be removed and destroyed. However, during an inspection of the medication rooms and carts, several expired medications were found. Specifically, three bottles of Vitamin B-6 with an expiration date of June 2024 were discovered in the [NAME] Station Medication Room. Additionally, the medication cart used by a Medication Aide Certified (MAC) at the East Station contained six cards of expired as-needed medications, including Cyclobenzaprine, Benzonatate, Clonidine Hydrochloride, Tizanidine, and Ondansetron Hydrochloride, all with expiration dates ranging from October to November 2024. Interviews conducted with facility staff revealed a lack of adherence to the policy. An LPN acknowledged that expired medications should be turned in to the Director of Nursing (DON). The DON confirmed that the expired Vitamin B-6 should have been removed and admitted that some bottles were missed during the removal process. The DON also mentioned that an in-service training had commenced to address the issue of checking expiration dates on medications.
