The facility failed to ensure the Minimum Data Set (MDS) was accurately completed for two residents reviewed for MDS accuracy. For one resident admitted with chronic pain, fibromyalgia, a left upper leg fracture near the hip joint, and osteoarthritis, the quarterly MDS with an ARD of 02/05/2026 indicated the resident was on opioids but had not received a scheduled pain medication regimen. However, the resident had an order for a pain patch to be applied every 72 hours, and the February MAR showed the patch was applied on 02/01/2026, which was not reflected in the five-day look-back period used for the quarterly MDS. The MDS Coordinator stated that if a resident was on a pain patch, the medication regimen should be coded as yes, and the DON stated the MDS would need to be modified if it was not coded correctly. For another resident admitted with muscle wasting and atrophy, severe intellectual disabilities, dementia, and schizoaffective disorder, the quarterly MDS with an ARD of 12/24/2025 showed conflicting nutritional coding. The MDS indicated the resident had received parenteral/IV feeding, had a feeding tube, and received a mechanically altered and therapeutic diet, but also stated the resident had no swallowing disorders and no proportion of calories or fluid intake through parenteral or tube feeding during the seven-day period. The resident's care plan addressed nutritional problems, but the order summary showed a regular diet with regular texture and consistency. During interview, the MDS Coordinator stated the dietary section had been coded as if the resident had parenteral/IV feeding, a feeding tube, and a mechanically altered diet, and the DON stated the resident had not had a feeding tube for the past six years.
