Linen Shortage Prevented Regular Bed Linen Changes
Summary
The facility failed to provide Resident #1 with a safe, clean, comfortable, and homelike environment by not ensuring bed linens were changed on several days between 6/30/2026 and 7/30/2026. Resident #1 was an [AGE]-year-old female admitted on 08/26/2025 with diagnoses including unspecified dementia with mood disturbance, muscle weakness, and major depressive disorder, recurrent, moderate. Her admission MDS reflected a BIMS of 15, indicating she was cognitively intact, and her care plan identified an ADL self-care performance deficit with assistance needed for bathing and bed mobility. During observation on 07/30/2026, the laundry room had very little clean laundry, with no towels or bed sheets present. Linen closets on Hall 100 and Hall 400 had no towels and little or no bed linen, and the Hall 300 linen cart had only a few towels and washcloths and no sheets. Staff interviews reflected that the linen shortage had been an ongoing problem for about a month, and the DON stated the shortage had been a consistent problem since she began working at the facility in early July 2026. The HSK Supervisor and CNA staff also stated the facility had been experiencing shortages of linens and towels. Resident #1 stated her bed linen was supposed to be changed on shower days, three times a week, but that this did not happen when showers were missed because of the towel shortage. She reported specific days when her bed was not changed and said the situation bothered her. Her roommate’s family member stated there was no clean linen in the facility on 7/24/2026 and that they had to take linen home to wash and return it so Resident #1 could have her bed linen changed. The resident’s record also reflected missed showers on several dates, and the facility’s linen order placed on 7/27/2026 showed a delayed shipping date of 08/01/2026.
